CASE MANAGEMENT

When individuals or families encounter a crisis and find themselves worrying about meeting their basic needs, Shalom Austin Jewish Family Service is here to respond. We provide an array of services and resources for individuals of all ages. The case management team works with our volunteer department to assist with a variety of needs.

We provide:

  • Assistance in identifying and applying for available resources
  • Support in building confidence and skills to address life’s challenges
  • Assistance in coordinating and managing resources from ​our partner ​agencies

Older Adult Services

JFS staff provides many services to older adults and their families. Through case management and counseling, JFS offers support groups and resources, recommendations for rehabilitation care, and living arrangements when possible.

Financial Assistance

Financial Wellness

We offer a holistic approach that is client-centered and strength-based to address clients’ financial, emotional, and employment needs. We support our clients, help them define goals, and create a plan to reach financial stability through financial education. By providing clients with encouragement and accountability, they are empowered to work towards their goals.

Food Pantry

We serve the Jewish community and beyond through our food pantry. We offer monthly opportunities to receive food, and other personal and household items assistance. Contact our case manager to learn more about the program, and discuss details such as future pickups or deliveries, available within a specific radius.

Job Search Assistance

Shalom Austin Jewish Family Service partners with Project Empowering and Employing (Project EM), a nationwide, virtual job seeker support platform. Project EM gives job seekers equal access to employment opportunities, providing all the assistance and tools a jobseeker needs to find work – from job lists to resume writing to career counseling and more. We provide our clients with access to resources throughout the country from the comfort of our client’s homes.

Transportation

Need a safe, reliable ride? Shalom Austin Mobility offers transportation to and from the Dell JCC, JFS, retirement and rehabilitation facilities and medical appointments.

Nurse-Patient Advocacy Program

Don’t navigate health challenges alone! Our Nurse-Patient Advocate guides clients and family members/caregivers in easing the transition through challenging healthcare situations.

We know that physical and mental health are intricately connected. Shalom Austin Jewish Family Service therapists and case managers work closely with our nurse-patient advocate to offer holistic services to meet the various needs of our clients.
