*This includes adding / removing family members from your account, changing your membership type, updating personal contact information. 

• You may downgrade your membership once per 12-month period, but you can upgrade at any time. 
• If downgrading, please submit this form by the 15th of the current month to allow time to process before the next billing cycle on the 1st of the next month. Downgrades submitted after the 15th will be processed the month after next. 
• We can only upgrade or downgrade memberships on the first of the month. If you would like to upgrade midmonth, there will be an additional $20 fee for the partial month. Otherwise, your change will take effect on the 1st day of the next month. 
• If a form is submitted after office hours, on a weekend, or over a holiday, your form will be addressed on the next business day. 
• If you are temporarily adding someone to your membership, you will need to submit a second change of membership form to remove them as active from the account. 

Change of Membership Request Form

• Members may freeze their membership for up to 6 consecutive months in a calendar year for $30 per month.  
• Freezes are done from the 1st to the last day of the month; we are unable to prorate freezes. 
• Please submit this form 10 business days before the month in which you would like your freeze to begin. 
• Submission of this form is not a guaranteed change. All requests are subject to approval regarding membership policies.

• As per your membership agreement, completion of this form fulfills the requirement of a 30-day written notice. 
• Effectively, you will be responsible for one more billing cycles regardless of the day of the month you submit this notice. 
• Membership cancellation requests are processed at the end of each month. You will receive a confirmation when your request has been processed. 
• If you wish to rejoin in the future, you will be responsible for the registration fee. If you end up changing your mind between now and the end of the 30-day cycle, the last day of your membership, we are always happy to cancel your cancellation. And please know you are welcome to continue using our facilities through the last date of your active membership.
